I'm having problem with workflows because of 2 user profile with same username. Basically, i have this user format (i:0#.w|<Domain>\<UserName>) and all are working, then suddenly same username showed (domain\UserName) and now sharepoint 2013 workflows are having problem with this account and even notification email is not sending when started a workflow.
How do i get rid of user with format (domain\UserName) in Sharepoint 2013?
I have checked our AD and verified that there is only 1 account with that username.

If there are only few users try deleting the duplicate user profile from Central Administration, else write a powershell script to eliminate duplicate user profile. Refer to the following post for more information
For deleting User Profiles from SharePoint 2013 CA, you can do as the followings:
1. Open CA, click Application Management
2. Click Manage service applications under Service Applcations
3. Scroll to User Profile Service Application, click it
4. Click Manage User Profiles under People
5. Then you can find user profiles and delete user profiles

I have deleted my profile in sharepoint using Administrator account and refresh all App poll and sites. However, it seems that problem still occurs. Workflow is not working well, I couldn't receive notification from the Sharepoint server while all users can receive same thing with audit logs.
What does this mean?
I'm having the same problem. In the User Profile Service in CA, it shows me only 1 time. However in the site collection users list (_catalogs/users/simple.aspx) it shows me twice and they both point to the same MySite yet have different titles. I see another account in my farm with the same issue and he was the farm admin before I was. Looks like its related to site collection administrators or farm administrator accounts. Still haven't found a resolution. Its a real hassle when trying to test workflows that send emails.
I'm not sure if this is applicable, but had a similar situation the other day.
When I looked in the "All Users" group (<SiteURL>/_layouts/15/people.aspx?MembershipGroupId=0) I found a duplicate there.
I removed both entries and added the account back to the security groups. It fixed the problem.

I've found the problem in my scenario. Sometimes I use SharePoint Manager 2013 in order to set settings which are not adjustable via UI. When logging in, SharePoint automatically creates a new user "domain/username". I don't know how to avoid it, but I simply delete the just created user.
