Dear All,
I want to create a workflow with the below steps, would you please help me on this, thanks in advace.
S. No
Question with Agree/Disagree
If Relationship details are filled
Action Flow
1
All Agreed
No Relation ship
Submitter will get notify and workflow completes
2
Some Disagree
No Relationship details
Notification to approver with below options
a) Approved – Approved
b) Noted – Noted
c) Request for more information – send form back to
Employee or his/her line Manager, once employee is submitted,
Back to approver
d) Transfer – assign to next levels with dropdown by 1st approver - this is second level
Even second level will have same options as first approval process.

Relationship details
Notification to HR
Notification to some department
Notification to Submitterworkflow completes
4
Some Disagree
Relationship details
Notification to HR
Notification to first level approver with below
a) Approved - Approved
b) Noted - Noted

1. Create three columns in the list, like Question with Agree/Disagree, Relationship, Relationship details.
2. Create workflow 2010 as following picture shows.

Hi Emily,
Thank you so much your support and answer but my requirement is little different, please have a look at the below screenshot. Response columns are 10 and family members are 5 columns.
Point 1. If user selects agree from all 10 fields without filling family members. just workflow completes.
Point 2. If user selects one or more disagree/unsure without filling family members, send an email to one department. This dept has a right to send it back to the requester asking for more info, again requester submits the form with more info, once again depart lead will get notified.
Point 3. If user selects all agree with at least one family members, just workflow completes.
Point 4. If user selects one or more disagree/unsure with at least one family members, send an email to another department. This dept has a right to send it back to the requester asking for more info, again requester submits the form with more info, once again depart lead will get notified.
