Icon Malformation

Question

Windows Vista is not properly assigning/using application/folder icons.

Setting a specific extension's default icon does not work. At some times, it appears as it does, except hovering over the file will display it back in its original icon. Most icons have been replaced with "unknown extension" icons. This includes folders.

I was able to resolve the issue by switching my icon set to a different one and then back to the original Vista icons.
