  • Hi venks123,

It’s by design that it’s not possible to add attachment to the survey.

But there’s a workaround to accomplish your requirement.

    1. Create a custom list.
    2. When you want to response a survey, create a new item to the custom list, and attach a file to the item.
    3. Create a lookup field in the survey to the custom list.
    4. Edit the view for the survey so that the lookup field will display

    Hope that helps.

  • Attachments are added to list items, not lists. The Survey list does not appear to support attachements at all.

What you may want to do is use SharePoint Designer to edit the survey's overview.aspx or newform.aspx page and add a hyperlink to the document.

You would get more responses to this question if you edited your subject line. "Urgent query" will not get you a faster response, and many people will ignore it as it gives no hint as to the question.

  • Hi,

Survey lists do not support attachments. So you can use the way specified by Mike or else you can modify the schema of survey list placed at the location C:\Program Files\Common Files\microsoft shared\Web Server Extensions\12\TEMPLATE\FEATURES\SurveysList\Survey to include attachments.

However the second one will affect all survey list
