Azure: Customers in Ukraine are required sign the Microsoft Online Subscription Agreement RRS feed

    When I was getting a trial subscription for Microsoft Azure I got the following popup:

    Customers in Ukraine are required to print, sign and return to Microsoft the Microsoft Online Subscription Agreement (“Agreement”) promptly upon purchase of your subscription. If Microsoft has not received your signed, paper Agreement within three weeks after purchase, your subscription to the online service(s) will be terminated. Please send the Agreement to: Microsoft Ireland Operations Limited Atrium Building Block B Carmenhall Road Sandyford Industrial Estate Dublin 18, Ireland In addition, please read the entire Agreement below before accepting terms and conditions. Please click here to view and print the agreement.

    Instead, I took this: http://azure.microsoft.com/en-us/support/legal/subscription-agreement/?country=ua&language=en . However, I still have difficulties with filling this document:

    Order Number - what is the order Number and where can I find it?

    Bank Information Customer Block - I don't have all that bank information: Correspondent Bank, Swift Codes and so on. Are those fields mandatory? Can I be billed just via a credit card?

    In general, should I even fill and send this? In general it looks like an old thing that someone forgot to remove
