documents table sharepoint site

Question

i have a table containing the documents on a SharePoint site.

so far so good.

but all of a sudden , if i add a document , there is a record appearing in the table, but the name isn't showed. it is blank.

how can i make it that when adding a document the name of the documents appears also in the file ?

What's changed or happened? Did somebody change something either in the design of the database, SharePoint, or installed any updates? Does this happen to all machines?
