    Given that C++.NET is managed code, are malloc's and free's still used? Also, can pointers and custom linked lists be used in C++.NET?

    You can use unmanaged objects and pointers. However, if you want to include managed object into an unmanaged list which typically accepts pointer to the element, you must pin the managed object in order to obtain its address. The object then won't be moved or collected by GC unless it goes out of scope. Here are some articles on the subject.
