How to show checkboxes, radio buttons on the create/edit work item form?

Hi all,
Can we customize an existing workitem for example Task and add check boxes, radio buttons to the create/edit form of the task?
I have a requirement like depends on the selection [radio button or checkbox] i need to show/hide some other fields.

Please help me out that whether it is possible or not.
If possible, how to solve this? i know how to create custom controls in TFS from SDK but i don't know how to create and integrate these controls with the form.....
