User Fillable Form and Calculating tool RRS feed

  • Hello All, 

    I have built an internal tool for my company in excel. It consists of 10 questions, each with 2 dropdown options. A user responds to each of the question by selecting a dropdown option for each of the 10 questions. The dropdown option for each question is different.

    For eg,

    Question 1  -  "Region" has options: 1. USA and 2. Canada

    Question 2  -  "Home Type" has options: 1. Apartment and 2. House

    so on and so forth.

    Choosing option 1 vs option 2 for each question has a value assigned to it. 

    Once all options are selected, the sum total of all values are displayed as a total score. 


    I am trying to create this form on sharepoint. Such that users will be able to fill the form (answer the 10 questions by selecting dropdowns) and see the sum total displayed below. 

    I tried fiddling with Sharepoint Lists / Custom lists / Survey, but didnt help. Any suggestions ?

    Any other solution (MS forms, flow or anything else within office 365 will also help).
