Creating a form on Access which allows a user to specify a criteria with yes/no and combo boxes RRS feed

    I am trying to create a form which will allow users to search a contacts list by the publications they want to receive.  There are approximately 20 yes/no boxes for publications and 3 industry type selection lists.  I would like to create a form which allows the user to select the publication(s) they are sending out and the form will create a list of names and email address for those who want the publication.

    help!  im a very stuck and time is running out.
