Sharepoint 2010 Survey List -- Print List Item

Hi all,
I have a survey list and want print functionality in the view form. I have created a custom action in the sharepoint designer but its not showing on the display form. Its showing on the other regular list but not on the survey list. What should be the ribbon location Id? How to add print to display form toolbar? as shown in the below picture.
Please guide me the steps. I just want to print Survey Question and answers of one list item with good font size.

Hi sam,
How did you customize your custom action? Is there a “RegistrationID”? If yes, you need to change it to “102” since the survey list of SPListTemplateType is “102”.
