As far as I know that if you want to create both physical machine and virtual machines in Lab Center, you will need to add a standard environment:https://msdn.microsoft.com/en-us/library/ee390842.aspx.
If you want to create virtual machines that are managed by SCVMM, it’s better to
add them to an SCVMM environment instead.

Generally, I know that there have no any licenses require when you create a lab environment in lab center from MTM.

When you want to install and configure the test controller and test agent to run automated test case using lab environment, you can install the test controller on the TFS Server machine or in a client machine.
In addition, when we create a lab environment, the test agent will be installed and configured automatically on the remotely machine( the physical machine or virtual machines ) you add the remotely machine in the Machines tab.
