    Did you make any changes in Upload.aspx page using any designer app ?

    And is this for a single doc lib or for all doc lib ?

    If this is for a single doc lib then you can replace the upload.aspx page from the correct doc lib to affected one.

  • Hi raj,

    Firstly you need to identify the difference between the default document content type and the custom content type.
    So you can check a library based on the default document content type and create a folder to see whether it is two text boxes. If it wasn't, it indicates that the situation is caused by the custom content type. Then you need to check your content type to see whether there is some special setting.

  • No, we haven't made any changes using Designer. This is for all document libraries which are tied to custom content types.
