I would like to create a custom security permission to a custom list.
This is a custom list to store user late record with their two level approval (Line manager & HR).
Items list must be visible for each owner (user) and his manager plus all HR members only.
how to achieve this?

Hi Hussain,
In this list, you can stop inheriting permissions and remove permissions.
Then, you can create a workflow to set permission when a new item is created.

First, you must have permissions to administer permissions. If you don't have them, them you'll need them, or work with the person who has them.
- Create a new group.
- Assign members to the group
- Assign group to the list, granting the desired permission.
It is possible that you'll need to create a new permission, and assign custom permission levels to it. (Again, you'll need the right permissions to do this.)
When view the list, select List Settings / Permissions for this list.

It isn't a perfect solution but you can break inheritance on each item and then set the permissions individually with a workflow.
You may hit scaling issues if/when you have lots of individual security levels in the database, however O365 seems to handle them without blinking and i heard a rumour that 2013 was more resilient than 2010 anyway.

Skip the Group step, and just assign the person directly, then.
