    It appears to me that SharePoint does something like this.  I notice that there is no folder structure in a sharepoint site, however you can use standard looking URL's to access sub-sites and other items.  How do they do it?

    You can do URL Rewritting in ASP.NET pretty easy.  I have a couple of Blog Entries on URL Rewritting, and here you can review.  I am doing some research right now on the VirtualPathProvider.  The MSDN web site uses this to rewrite URLs.
