Cannot publish InfoPath forms to a load balanced url

Hello,
I am having difficulties while trying to publishing an InfoPath form to a forms library.
To find more, I created a blank web enabled InfoPath form and published this form to a new forms library, for the first publish life is good, no problems at all, the second time I tried to publish though I did no changes at all, I keep getting the below message.
I launched fiddler, and there I see 432 status returned.
Note, that I am working in a load balanced environment with two WFE. When I publish to another web app that doesn't use this load balanced url i.e. directly to server I got no errors.
Please help.

Please
- Make sure you have permission to design the form template for related form library.
- Check whether the form template is checked out by anyone.

Thanks for your help.
I've went over the point you've listed, all of these are already covered though the problem persists.
ULS doesn't have much more to add it just says
#90040: The file "http://intranet.../test2/Forms/template.xsn" is locked for exclusive use by win2k\mmg33.
