Business Rule: OR Condition acts like AND

Question

I have the following condition:

IF IsApproved = N OR IsApproved IS BLANK THEN IsApproved = NOT VALID.

I have entered this Business as follows (see image below) however, the Action Is NOT Valid does not fire.

When i enter the conditions separately, it does work but not when i combine them in one condition with the OR statement. Can someone tell me whether this is a bug or is me?

You will need to start with an OR from the Logical Operators then add your conditions.  See image below.
