Diagram Tool To Create Database Design Diagram to help Optimize database hardware performance

Is there a tool I can use to map out database physical design like, where the RRSR report server are located, where each database files are? IO bottle neck etcs?

There is no single tool. You need a good DBA. SSMS is the best tool for server & db management. SQL Server Profiler & Windows PerfMon are the main tools for diagnostics.
