Approval Process Task form

Question

I've had an "Approval - SharePoint 2010" reusable workflow working for several weeks. In SPD 2010, I added in the 'Tasks Form Fields' an existing column.

Now when I open up the task form as the participant (approver) it is blank except for two links: 
"X Delete Item";
"O This workflow task applies to 'itemname'. ";

Any suggestions?  Thanks
