How to obtain Intercept and Beta coefficients of Logistic Regression Model

Hi,

I am currently trying to create a Logistic Regression Model relating to healthcare data and the purpose is to create a table which consists of the Input column, the beta coefficients and the intercept in order to create a simple formula to assess risk of admission. This table will be used to calculate the risk of that individual patient manually, similar to that of a scorecard method.

In summary i want to know how to view the intercept and beta coefficient values.
