Dynamic Initiation Forms

Hi
I need to create an initiation form that has a series of checkboxes. Each of those check boxes is a cell of a column in another list, however, that list may grow over time, and so in the initiation forms, there'll be more checkboxes in the future. What I want is for each time the initiation form fires, it checks the values in that column, and creates as many initial parameters, as the cells in that column, and use the text in those cells to be the label of the parameter.
Is this possible to do with Sharepoint 2010?

There isn't any way to make the names of check boxes dynamic like that, I guess unless you want to try and delve into custom code.
