Is it possible to autoscale load balanced Virtual machine Instances like AWS ?

Not directly. Virtual Machines need to be manually managed. You can set up multiple machines, but there's no autoscaling capabilities provided by the Windows Azure platform. You are responsible for making the determination when additional instances are needed and then spinning them up.

However, there are third parties such as Rightscale and ScalExtreme that will provide some of this functionality for you.
