I would like to know how Workflwo Manager 1.0 is licensed in both Azure and On-premises.

Is it provided as part of the base framework or as a separate or pay-per-use component?

Workflow Manager 1.0 is currently available only as an on-premise product. It is a separate download and it is free.

The End-User License Agreement is the license. You can also refer to this blog post:

BTW, it is also supported in VM Roles (Azure Virtual Machines)
