can workflow Initiation form text label show List Item field?

Question

Is it possible to show List Item information in the Initiation form of an associated work flow?

For example, say I have a list called ECNs (Engineering Change Notices).  One of the fields in this list is: Part_ID.

When someone manually starts a workflow off of one of these list items, I want the INITIATION form to show the Part_ID.

Seems easy right?  Is there a way to do this?

Please please please please help me.

Seems we cannot display list item properties in the list workflow initiation form via SharePoint Designer since the form is loaded before the workflow start and there is no a task to perform a workflow lookup for the item property . It might require some programming effort.
