Infopath 2007, new form is created again and again when submitted

Question

I have created an Infopath 2007 form. It has 3 sections; First is for the submission of the form and other two for the reviewing. After the form is submitted, unique ID is generated from the Nintex workflow (2007). Reviewers have read access to the submitted first section but can edit their (second and third) sections. I have created a Submit button using Rules in the infopath form.
Now the problem is whenever the reviewers submits the form after reviewing the form, new form is created again and again. Need your suggestions.

Would you please tell me where you place your form? Is it in the SharePoint, Outlook or other place?
