How to get the PersonID and RecordID during sign up

I created an asp .NET Linking Applications  to offer patients a one-time "setup" I am trying to get patients to link their HealthVault account/record to their application account. During this initial 

office visit connection to HealthVault during this first time visit the user will signed up for HealthVault and authorized my application. After signing up and before leaving Healthvault How would my application request the user‟s HealthVault PersonID and RecordID. I know what the cdode should be, but at what point do I get the information. I need to do it before the user exits Healthvault and after the sign in is completed.Just to be sure I am linking correctly, what is the https link fo linking to Healthvault.
