I've put a suggested data type for each.
Date of HCV Hepatitis C Virus diagnosis (Condition)
Suspected ROI: Suspected Region Of Interest (Not sure. I would need more context about this information)
PMH: Past Medical History (Condition for personal, family history for others)
Psychiatric History/Diagnosis and current CESD (condition for simple stuff. I don't know enough about CESD)
Substance Use, (question answer)
Smoking history (question answer)
Current Medications (Medication)
Vital Signs with Height Weight and BMIAbnormal physical findings, especially signs of hepatic decompensation (spider angiomata, palpable spleen tip, ascites, peripheral edema, asterixis or other signs of hepatic stigmata) (vital signs can go in vital signs, height and weight in Height and Weight types. BMIabnormal sounds like Condition to me, as do the others that you list)
CBC Complete Blood Count with diff. CMP cytidine monophosphate, (Lab test results)
Hepatitis A Serology, (Lab test results)
Hepatitis B Serology, (Lab test results)
Lipids, (Lab test results)
PT/INR Prothrombin Time Internal Normalized Ratio, (Lab test results)
Uric Acid (Lab test results)
