Strange SharePoint user issue on 2010 version - any suggestions?

Question

We have one SharePoint user reported his alert email is different from his email account.

His correct userID is “NA\userID” and the email should be [e-mail] as indicated in AD. However the email looks [e-mail] that is different from the correct email. In addition, we check the alert on the site, the alert displayed is “userID_alias (NA\user_alias)” instead of "userID (NA\userID)". The people picker displays the different search result from different webapps if we search userID.

Are you using User Profile Service, what does it shown on his profile in there?

Everything looks fine on the profile. Same as we can see from central admin and My site.

Have you ever changed the user's email to "[e-mail] " before?

Do you have more than 1 smtp address conntected with the userid in Exchange?  Is it possible that something at the exchange level is defaulting to a primary address?

Failing that, has the user experienced a name change recently?  Or has the domain name changed?
