Please help me find my workflow forms, association and task forms not displaying

Good day all. I sincerely hope you can help me solve my problem.

I have a workflow based on the OOTB 'Approval SharePoint 2010'.  I've customized the task and the association forms in InfoPath.  They worked until I tried to add a custom field to the 'Tasks Form Fields'. After which the wrong forms are loaded for the workflow.

When I try to associate for workflow, or assign a task using the workflow, the forms are empty.

I suspect that it has to do with the task and history lists, but i don't know how to fix this.

Maybe I can explain this better with pictures :) 

I have an approval workflow...
and the association form looks like this in InfoPath...
When I try to associate the workflow...

I get the create workflow questions...
When I click next...
I get a blank association form...

I'm not totally familiar with SharePoint 2010, but I don't believe this is proper behavior.

Any experts?

Hi,

What is the relative path to your association form? The url in your blank form screenshot you posted.

Also, did you publish the for after you edited in InfoPath designer?

Hi James,

The path of the association form is 'The site name' followed by:

_layouts/CstWrkflIP.aspx?List={guid to the list}

where the guid is for the list I'm trying to attach the WF to. It's not possible to attach as there is no 'Submit Button' on the form. see image in previous post.

Yes, I've published the form in Infopath and the workflow in SPD
